Job Description:

The Social Worker will be responsible for reviewing patient charts and communicating with the Clinical department and providers around any irregularities as part of chronic care management.

Responsibilities:

• Provides assessment and care management services, including:
• The administration of validated rating scales
• Initiates behavioral health care planning concerning behavioral or psychiatric health problems
• Revises/modifies care plans for patients not progressing or whose status changes
• Brief psychosocial interventions if needed
• Ongoing collaboration with the billing practitioner
• Maintenance of the registry/tracking sheet C
• Consultation with the psychiatric consultant
• Has a continuous relationship with the patient
• Available to deliver services face-to-face with the patient
• Collaborative, integrated relationship with the rest of the care team
• Conducts an interdisciplinary care plan meeting to go over the patient’s beneficiary to the program

Qualifications:

• Bachelor's degree in social work
• Experience in long-term care preferred
• Experience in behavioral health preferred
