Role:
As a Hospice Patient Care Manager, you will oversee the entire care team and ensure the delivery of exceptional care to our hospice patients. You will coordinate care, manage resources, and provide guidance and support to our dedicated team of caregivers.
Qualifications:
· Current RN license in the state of Florida with a minimum of 3 years of clinical nursing experience, preferably in hospice or palliative care.
· Proven leadership and management skills, with experience supervising and motivating a multidisciplinary team.
· Strong clinical assessment and critical thinking abilities, with a focus on providing patient-centered care.
· Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with patients, families, and team members.
· Compassionate and empathetic approach to patient care, with a commitment to upholding the highest standards of professionalism and integrity.
· Proficiency in electronic medical records (EMR) and computer skills.
Responsibilities:
· Manage and supervise the interdisciplinary care team, including registered nurses, certified nursing assistants, social workers, chaplains, and volunteers.
· Coordinate and oversee the delivery of comprehensive care to hospice patients, ensuring that their physical, emotional, and spiritual needs are met.
· Conduct regular assessments of patient needs and develop individualized care plans in collaboration with the care team and the patient's healthcare providers.
· Provide clinical guidance and support to the care team, ensuring compliance with all regulatory requirements and best practices in hospice care.
· Serve as a liaison between the care team, patients, families, and external healthcare providers to facilitate communication and ensure continuity of care.
· Monitor patient outcomes and quality of care, implementing strategies for continuous improvement as needed.
· Provide education and training to staff members on hospice care practices, policies, and procedures.

Physical Demands:
· Ability to stand, sit, walk, reach with hands/arms, lift, bend, and balance.
