A growing Healthcare Group is seeking a Licensed Practical Nurse (LPN) to work as a Care Coordinator for the Chronic Care Management (CCM) program while working out of an office located in Hollywood, FL.

Responsibilities:

• Review patients' medical records to ensure that their plan of care is being followed and that patients' medical management needs are being met.

• Your responsibilities will include chart audits and reviews, medical charting, maintaining a patient registry, and attending care team meetings.

Requirements:

• Valid LPN certificate/degree in FL

• Basic healthcare background experience

• Ability to work full-time

• Detail oriented

• Effective time management skills
