Maury Regional Health is Hiring a Social Worker Near Columbia, TN

Title: Social Worker - Care Management

Location: Maury Regional Medical Center

Position Shift: Full-Time

Position Summary:

The social worker evaluates a patient’s social, emotional, environmental, financial, and support needs. Assesses, plans and acts in response to the psychosocial needs of patients/families. Intervenes in crisis situations to assist patients. Facilitates decision making on behalf of patients and families in complex cases. Collaborate with care manager in assessment and coordination of discharge planning. Provides interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Responsibilities:

  • Conducts comprehensive psychosocial assessments of patients with documentation in the Electronic Medical Record.
  • Arranges all available resources to ensure patient achieves optimal well-being, including transportation, medication assistance, food assistance/boxes, DME, shelter, community resource information, and Foundation requests.
  • Assesses for Advance Directives, providing education and assistance with application and completion as needed.
  • Focuses efforts on reducing preventable readmissions through the promotion of patient coordination, discharge planning and collaboration with healthcare team while promoting optimal patient outcomes.
  • Identifies patients with underlying mental illness, behavioral health, or substance abuse concerns, providing resources and making referrals as indicated.
  • Provides crisis intervention for various patient situations, including psychiatric emergencies, elder abuse/neglect, rape, child abuse/neglect/drug positive and domestic violence. Will report per legal mandate abuse and neglect to the appropriate agency (APS, DCS). Complete referrals for inpatient psychiatric treatment per completed 6404 and physician order.
  • Provide initial grief counseling/screening with patients/families/support system and provide ongoing community resources.
  • Provide support for adoption cases through assessment of birth parent, providing community and counseling resources as needed. Provide resources for adoptive family as needed. Coordinate adoption paperwork with attorney and consult with compliance as needed.
  • Provides education to patients regarding illness and treatment options, facilitates communication between patient, family, and healthcare team through family care conferences, progression of care rounding and individual meetings, remaining an advocate for patients and families.
  • Work with patient, family, support system, and healthcare team to coordinate an individualized discharge plan, to include assessments, referrals, and community resources needed.

Credentials / Education:

Preferred: Licensed Social Worker Active applicable state licensure

Preferred: 1 yr. Social Services experience in Hospital

Required: Bachelor’s in Social Work
