Master Center for Addiction Medicine is Hiring a Community Liaison Near Kilmarnock, VA

We are seeking a Community Liaison to work at Master Center for Addiction Medicine! We are a private, outpatient addiction medicine treatment center headquartered in Glen Allen, Virginia. We have additional offices in Hampton and Gloucester and we are expanding to provide services to other communities in need, particularly rural communities, where addiction treatment may be difficult to access. Our approach to addiction is comprehensive, evidence-based and individualized, drawing on every tool that has been proven effective: medical treatment, education, psychiatry, professional counseling, testing/monitoring and peer support services. Our aim is not just to treat patients but to heal them – by helping each one find his or her way to lifelong recovery.

The Community Liaison works closely with the Administrative Team, the Director of Operations, Client Development, and the Coaching Team. The Community Liaison is committed to creating important relationships with referral partners within the community, providing direct coaching services to patients and will also oversee some basic office management responsibilities

This office has not yet been opened but we are actively hiring for this role and have an immediate need. Once hired, the Community Liaison will work remotely until the office is opened. It is important that the Community Liaison create, develop and foster relationships within the community prior to the physical space opening. This is a great fit for anyone with experience in or interest in the fields of addiction and recovery, healthcare, medical offices or mental health. If you have sales, hospitality, restaurant or customer service experience and love connecting with and speaking to people, this would be an excellent position for you!

The job requires keen attention to detail and the ability to multitask combined with patience, empathy, and respect for others. This is a position that will involve a significant amount of time in-person and remotely with referral partners and patients, and it requires the ability to communicate effectively in-person, over Zoom calls and over the phone with patients, as well as the ability to communicate effectively with other team members regarding patient care, workflows and operational aspects of the job.

We do candidate assessment batteries. You are required to complete the competencies. If you do not complete the assessments, the hiring manager will not schedule an interview with you after the initial phone interview.

JOB TITLE: Community Liaison

LOCATION: Kilmarnock

IMMEDIATE SUPERVISOR: Director of Client Development

DESCRIPTION:

The Community Liaison will develop and maintain relationships with community resources in the region, including therapists, sober living, law enforcement, hospitals, medical providers, and others that have a need to refer to substance use disorder (SUD) treatment. This individual will represent The Master Center for Addiction Medicine in the community and will need to serve as a liaison with multiple departments: outreach, healthcare navigation, peer recovery coaching, therapy, medical team and care coordination.

The Community Liaison will provide peer recovery coaching for the induction of new patients in the local community. Other responsibilities include patient engagement, peer support, accurate documentation in Master Center’s EHR system, care coordination, and assistance with referrals. This position may also require minor office management duties, such as supply inventory, working with building management, and maintaining a safe environment for staff and patients.

RESPONSIBILITES INCLUDE:

Community Liaison:

  • Establish and maintain referral relationships with therapists, law enforcement, hospitals, medical providers, and others that need to refer for SUD treatment.
  • Schedule presentations with appropriate resources on Master Center’s model of care and services.
  • Host and attend local events to network to represent Master Center.
  • Serve as primary contact for referrals in the region, coordinate with Richmond-based team for insurance verification and patient scheduling.
  • Document referral partner contact in Hubspot, coordinate with Outreach and Marketing on various campaigns and other outreach efforts.
  • Maintain detailed understanding of all Master Center programs to communicate accurately and effectively with referral resources.

Peer Recovery Coaching Specialist:

  • Serve as peer recovery coach throughout the week as needed for in-person inductions and follow-up appointments.
  • Conduct coaching interview during patient appointments, which may include the following components

o Review a patient’s story, focus on crisis that prompted them to seek treatment

o Review a patient’s recovery history, depth of understanding

o Assess a patient’s readiness for change, openness to treatment

o Help patients identify obstacles to their recovery

o Help patients set treatment and recovery goals

o Provide support to patients in adhering to their treatment plan

o Describe the role of the coach in a patient’s treatment plan, including methods of communication with patients

o Provide reports to the medical provider

o Provide support to the medical provider to ensure appointments stay on schedule

  • Serve as the first point of contact for patients between their appointments. The coach must respond to voicemails, telephone/web encounters, or other methods of communication in a timely manner.
  • All coaching documentation should be entered into the electronic health record, in a detailed, accurate, and timely manner.
  • Provide support with some medical assistant responsibilities, when needed. These tasks are documented within Master Centers electronic health record and may include the following:

o Recording vital signs

o Collects, documents, and reports Urinary Drug Screen, ETOH breathalyzer and mouth swabs.

o Completing pregnancy tests and BACs.

  • Facilitate care coordination, both within the practice and with outside providers/partners, as needed.
  • Act as a liaison between the provider and patient for communicating medical decisions to patients.
  • Responsible for monitoring patient engagement, following up with those patients that may be at-risk, have no showed for their appointments, or that need to be discharged
  • Complete necessary letters or notes for patients, as needed, and respond to faxes.
  • Operations and conduct guided by all Master Center Standard Operating Procedures and documentation requirements.

Office management:

  • Work with medical staff on-site and coordinate with Richmond staff to purchase office or medical supplies as needed, and maintain a supply of printed material like patient agreements, ROI, etc.
  • Serve as primary contact with landlord for coordination of schedule or other issues.
  • Coordinate with Richmond team as needed.

Special Projects as needed and other duties as assigned.

MINIMUM QUALIFICATIONS:

  • High school diploma or equivalent.
  • Current Peer Recovery Specialist certification preferred.
  • 3-5 years business development or community outreach experience.
  • Local to Kilmarnock or surrounding counties.

This position is Monday through Friday 830AM to 5PM. Some weekend and evening hours may be required for business development tasks.
