Maryville Addiction Treatment Center is recognized as a leader in the field of substance use and co-occurring disorders. For more than seven decades, our holistic, evidence-based approaches have helped bring hope, health and healing to those suffering from opioid, alcohol and other addictive substances. Due to our accomplished staff’s education, experience, compassion and commitment, clients who have been served by Maryville consistently document high satisfaction levels with our programs and services.

The Care Manager assists clients as they transition throughout the continuum of care by matching their identified needs with available resources, and then assisting them to access care and services intended to meet those needs. The Care Manager works with clients to implement services and strategies that have been identified in their client’s individualized care management plans.

Responsibilities:

- Develops, implements and revises a care management plan to guide care

management interventions. Plans must be included as part of the full treatment plan

and updated per applicable licensing regulations.

- Engaging clients to be full partners in planning their own treatment and recovery.

- Collaborating with clients to identify activities that will help them maximize their

opportunities for successful community living.

- Providing facilitated linkages to medical, behavioral health, housing and community-based recovery services including Medication Assisted Treatment (MAT) and

coordinated care for HIV and Hepatitis C.

- Coordinating necessary services with the recipient’s insurance when applicable.

- Providing therapeutic care, treatment, and rehabilitation of clients.

- Works with other social agencies and managed care organizations, hospitals, clinics,

courts, prisons, and gatekeepers to meet client needs and assist them in utilizing

provided resources.

- Provides assessments & evaluations based on American Society of Addiction Medicine

criteria.

- Assesses treatment plans for clients using the Addiction Severity Index and the DSM-5.

- Determines the appropriate level of care according to ASAM criteria.

- Performs other related responsibilities and duties as required.

Qualifications:

- Bachelor’s degree in a behavioral health-related field is required.

- Knowledge of the basic principles, philosophies, practices, and procedures involved in

treatment of substance abuse and co-occurring mental health disorders is required.

- Must be able to develop a therapeutic relationship with patients while maintaining

therapeutic boundaries.

- Microsoft Office (Word, Excel, Outlook) is required.

- Excellent organization, written and oral communication skills is required.

- Must have the ability to take initiative and use innovative techniques in preparing

documentation.

- Must have the ability to establish and maintain effective working relationships with

clients, other members of staff, outside agencies, institutions, and the general public.

- Must possess excellent time management skills to plan, prioritize, and coordinate

multiple projects.

- Valid driver’s license, insurance and clean driving record is required. Travel up to 25% between various Maryville, Inc. sites.
