Job Summary:

We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for coordinating and managing care services for our clients, ensuring their well-being and satisfaction.

Responsibilities:

- Coordinate care services for clients, including scheduling appointments, arranging transportation, and ensuring medication adherence
- Conduct assessments to determine clients' needs and develop individualized care plans
- Communicate effectively with clients, families, and healthcare providers to ensure continuity of care
- Monitor and evaluate clients' progress and make adjustments to care plans as needed
- Provide direct support to clients with activities of daily living (ADLs) such as meal preparation, personal hygiene, and mobility assistance
- Uphold resident rights and maintain a safe and clean environment for clients

Skills:

- Experience in home care or senior care settings
- Strong organizational skills with the ability to manage multiple tasks efficiently
- Proficiency in driving to assist with client transportation needs
- Knowledge of care systems and procedures
- Ability to provide direct support with compassion and empathy
- Familiarity with meal preparation and dietary requirements
- Understanding of ADLs (Activities of Daily Living) and caregiving principles
