PHYSICAL THERAPY ASSISTANT (PTA) - HOME HEALTH
FULL TIME OR PART TIME

Summary

To provide skilled Physical Therapy services, on an intermittent basis, to patients in their homes, under direction of the Registered Physical Therapist.

Essential Job Functions: Responsibilities and Standards

  • Under the direction of the Physical Therapist (PT), provide patient services, which have been delegated by the PT.
  • Under the supervision of the PT, assist in evaluation and development of a rehabilitative plan of care and in periodic re-evaluation as required.
  • Participate in the preparation of clinical and daily progress notes; all notes are reviewed by the PT and submitted within 48 hours. Also prepares periodic summary reports.
  • As directed, perform routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
  • Together with the PT, instruct the patient and family and assist in teaching exercise programs to the patient and family.
  • Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies and contra-indicated medication and report to the PT.
  • Observe and record activities and findings in the clinical record and reports to the PT and the physician the patient’s reaction to treatment and any changes in the patient’s condition.
  • Consult with the PT regarding the outcome of the home visits and report physical findings.
  • Instruct other health team members including as appropriate, HHAs and family members, with respect to certain phases of physical therapy so that they may work with the patient.
  • Attend Rehab department staff meetings and case conferences.
  • Participate with staff, patients and physicians in discharge activities and assist the PT in completing the Physical Therapy Discharge Summary.
  • Confirm the scheduling of visits with your supervisor in order to coordinate agency visits.
  • Assume responsibility for self-development by continually striving to improve your health care knowledge through education programs, attendance at workshops and conferences, active participation in professional and related organizations and individual research and reading.
  • Assume other duties as assigned by PT, Nursing and the Executive Director.

ASSISTS IN THE AGENCY ADMISSION PROCESS

  • Obtain medical history, particularly as it relates to the present condition.
  • Evaluate the patient and the patients home situation to determine what instruction will be required, what assistance will be available from the family in caring for the patient and what other Agency and community services will be required.
  • Ensure that the physician’s orders are appropriate and discuss necessary changes.

Physical Requirements

  • Must have a valid driver’s license and be willing and able to drive to patient’s residences.
  • The ability to access patient’s home which may not be routinely wheelchair accessible is required. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform and demonstrate patient care.
  • Physical activities will include walking, sitting, stopped and standing and minimal to maximum lifting of patients and the repositioning of patients.
  • The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing in English is required.

Knowledge, Skills, Experience Required

  • Must be licensed as a Physical Therapy Assistant in the state of MA.
  • Associates Degree from an APTA accredited program.
  • Must have current CPR certification.
  • Strong oral and written communication skills, organizational and analytical skills required.
  • Ability to function efficiently in multi-faceted systems and to interact effectively with all levels of staff and customers.
  • Demonstrated ability to utilize independent and effective clinical judgment, problem-solving and decision making skills.
  • Utilizes effective time management skills.
  • One or more years of experience in a home health agency is preferred.
