PAY RANGE: Licensed Salary Schedule, $57,000 - $118,395 Annual Salary
WORKDAYS: 190 Days
REPORTS TO: Executive Director of Integrated Services or Designee

JOB SUMMARY:
The Transition Coordinator assists students with disabilities and their families in obtaining services necessary to transition from high school to a post-secondary setting, whether for post-secondary education, employment, or long-term support.
QUALIFICATIONS:
  • Current licensure for Special Education Generalist
  • Current licensure for Principal (Type D), Preferred
  • Excellent communication and interpersonal skills
  • Experience with creating new programs or models.
  • Strong understanding and experience with web applications including but not limited to Microsoft Office 365, and expertise with various digital tools, including online education.
  • Self-directed learner with strong problem-solving capabilities with capacity to learn independently.
RESPONSIBILITIES:
  • Provides direct instruction to students with disabilities that is designed to meet their individual transition needs.
  • Develops curriculum, designs, and implements lesson plans, and assesses student progress.
  • Disseminates information to staff, students, and parents regarding student needs, progress, and resources.
  • Coordinates transition activities for college, career, and community readiness
  • Manages IEPs for assigned caseload; coordinates and facilitates IEP meetings, writes IEPs, and communicates with general education teachers and support staff.
  • Facilitates referral, application, eligibility process for student and families to adult service providers.
  • Assist with alignment of transition continuum of services at the secondary level.
  • Attends IEP meetings upon request.
  • Performs other related duties as assigned.
NOTE: Special Education staff are hired and assigned to schools and programs based on the level and type of student needs. Staff members may be reassigned or released in accordance with applicable law and policy, when any of the following occur: program modification, enrollment decline, or change in level and/or type of student needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Understanding of disability areas under IDEA.
  • Knowledge of how disabilities impact an individual’s ability to function in the post-secondary setting.
  • Knowledge of state and federal laws/regulations that protect individuals with disabilities in education, employment, and community.
  • Knowledge of district policies/procedures.
  • Knowledge of adult service providers and services available within each.
  • Demonstrate a willingness to collaborate, solve problems.
  • Establishes and maintains strong classroom management.
  • Provides expanded opportunities and support for learner success.
