Job Summary

The RN Case Managers responsibilities involve delineation of value-added care management processes and the coordination of continuum of care services in a cost-effective manner necessary to ensure optimal patient outcomes.

What You'll Do:

• Ensure the patient in the appropriate status and level of care for the patient's clinical condition and escalate to the Physician Advisor for secondary review when deemed warranted.
• Contribute to decision making and decision support as a member of the interdisciplinary team while sequencing the plan of care.
• Assess patients for clinical, psychosocial, financial and operational factors that may affect the progression of care and implement timely discharge plans based on advocacy, patients' clinical conditions, personal choices and available resources when identified at risk by the nursing or medical staff or by case management screening.
• Arrange/ensure all elements of the transition plan are implemented and communicated to key stakeholders including, but not limited to, the health care team, patient/family/caregiver, and post-acute providers for continuity of care and a safe discharge.
• Assure prudent utilizations of all resources (fiscal, human, environmental, equipment and services) balancing cost and quality to ensure optimal clinical and financial outcomes.

What You'll Need:

• State of California Registered Nurse license required.
• American Heart Association BLS certification required.
• Experience in Case Management/Utilization Review preferred.
• Obtain Accredited Case Manager (ACM) credential within two years of full time employment with one-hundred percent compliance.

Job Information:

• Full-time, day shifts, including weekend day coverage.
• Starting wage $54.96 to $60.79 based on qualifications.
