MaineHealth is Hiring a Case Manager - Community Integration Services Near Portland, ME

Summary

For a limited time MaineHealth is offering up to $5,000 Sign on bonus for all eligible Case Managers!

  • Eligible candidates are hired (offer accepted) into a Full or Part time Case Managers position. Bonus amount prorated for Part time hires, per diem hires are ineligible.

Position Summary

This position is working with our grant-funded Community Integration clients in Cumberland County. 

Our program offers:

  • Excellent training to include weekly meetings with your supervisor, a go-to case manager for support with questions, and opportunities to shadow other case managers. 
  • Two program managers on site, to provide clinical support and answer questions. 
  • Flexible work hours and scheduling
  • 14 case managers, many of whom have been part of this team for more than a decade and who are able to help one another with questions about resources and client care.
  • Access to therapy and psychiatry for our patients, allowing our case managers to easily connect with other members of the client’s care team. 

The Case Manager is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. The Case Manager will ensure that client service delivery practices are ethical, culturally-competent, respectful of diversity and inclusion, welcoming and geared to customer service satisfaction. The Case Manager will be compliant with regulatory, accreditation, contract and best practice standards. The Case Manager will provide Community Integration Services within Cumberland County.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor’s degree from an accredited institution with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation or closely related field is required.
  2. License/Certifications: Eligibility for LSW licensure preferred. MHRT/C is required. Valid Driver’s license and appropriate level of automobile liability insurance.
  3. Experience: 1 year of experience in the same areas of specialization. 2 years of experience working with a broad spectrum of clients with mental health diagnoses is required. Prior experience with clinical methods and techniques used in supportive counseling as related to mental health and/or substance abuse issues is required.
  4. Excellent computer skills are required: Microsoft Word, Excel and use of electronic consumer record software
  5. Knowledge and commitment to abide by the Code of Ethics of the National Association of Social Workers
