Madison Regional Health System is Hiring a RN Case Manager Near Madison, SD

Madison Regional Health System is seeking a part-time RN Case Manager to be a part of the Madison Regional Health System mission: Committed to providing quality healthcare, close to home.
Our RN Case Manager is an essential part of our Medical team and is responsible for facilitating the plan for an orderly transition of patients from the acute care setting and transition to discharge. They are responsible to plan, develop, organize, implement, and evaluate all aspects of discharge planning. Implements services to patients and families; assuring that the medically related emotional and social needs are met/maintained on an individual basis. Maintain contact with referring agencies and for resources that are available. Excellent customer service, attention to detail, great telephone etiquette, computer knowledge, and the ability to multi-task in pressure/stressful situations are essential. South Dakota RN license and CPR certification is required.
The RN Case Manager part time position is eight hour shifts. The working conditions will be in a clean, well-lighted area, with sitting, standing, walking, bending, stooping, frequent repetitive motion of the hands/wrists involved. May be required to deal with unpleasant environmental situations resulting from patient care or patient activity. Exposure to infectious disease, body fluids, odors, and dust.
Part-time positions come with Madison Regional Health System's benefits package and a competitive salary. MRHS offers retention pay to individuals who have worked at the facility for greater than one year. Madison Regional Health System employees receive a discount at the on-site cafeteria.
