Friday through Tuesday
3pm to 11pm
Principal Responsibilities:
Manage a caseload of requiring utilizing a Rapid Rehousing model under a Housing First approach.
Engage in rapport building with participants and provide service from a “choice” perspective while utilizing participant-centered, harm reduction, and strengths-based approaches
Provide case management services including assessment, budgeting, employment search, and creation and monitoring of housing plan. Refer/connect participants with appropriate services such as financial assistance, entitlements, addiction services, advocacy, employment assistance, financial counseling, rental assistance, deposits, Domestic Violence services, Mental Health services, etc.
Document case management services through detailed and thorough case notes in Clarity for each Individual to include: weekly meetings, client and collateral contacts each week, as well as any significant events and/or interactions.
Enter data into HMIS/Clarity that complies with data integrity and data quality standards.
Connect with landlords and realtors to familiarize them with the RAFT/RTI housing programs and create a network of housing opportunities.
Accompany clients in housing search and outreach to available housing options.
Apply knowledge of residential lease contracts and landlord laws to educate clients of their rights and responsibilities
Relevant experience will be considered in lieu of a degree.
Qualifications:
*Experience working with people from very diverse backgrounds
*Excellent communication and conflict resolution skills
*Ability to implement and utilize limit setting techniques and appropriate boundaries
*Ability to speak, read and write English, bi-lingual preferred
*Ability to pass CORI/background check
