We’re currently seeking a part-time Social Worker to serve residents in our short-term rehabilitation program at Spang Crest, a Luthercare community in Lebanon, PA. The position is Full Time 8am-4pm, Monday through Friday.

WHAT YOU DO:

As a Social Worker at Spang Crest, you’re an integral part of the team who supports residents in our short-term rehabilitation program. The ideal candidate has experience in long-term care, discharge planning and has a good knowledge of community resources. In the position, you would do the following among your duties:

• Obtain social and medication history, financial information, and other relevant information required for the admission process. Provide information and explain the admission policy and process to prospective residents, families and physicians, including details about our programs and costs and Medicare and Medicaid determinations.
• Help residents adjust to living in our community as well as assist residents and their families with discharge/post-discharge plans and applying for medical assistance.
• Participate in coordinating care services for residents with interdisciplinary teams, and monitor hospitalization of residents with family members or appropriate hospital personnel for any change in placement status.

QUALIFICATIONS/REQUIREMENTS:

• Bachelor’s degree in social work
• A valid PA driver’s license

Spang Crest, located at 945 Duke Street in Lebanon, is a 105-bed skilled nursing and rehab center. The community is part of Luthercare, a not-for-profit, faith-based, and mission-driven organization based in Lititz, Pa. Luthercare provides a network of services through its full continuum of care with three communities in Lancaster and Lebanon Counties.
