Minimum Requirements

  • 5 years of recent experience as a Director Of Nursing Services in a Skilled Nursing Center (SNF)
  • 7 Registered Nurse

About the RN-QAPI position:

This position will report to ED, DNS and Regional Clinical Director.

This Position is responsible for clinical projects, quality assurance projects, documentation, and other assigned duties in assigned nursing Center.

·Position is responsible for collaborating with clinical/facility staff to assure that all Center and management policies/procedures are followed, documentation is complete, timely, and accurately reflects the patients’ condition and care provided and that clinical outcomes are assessed and reported. Educates and mentors staff on quality initiatives.

To develop and maintain effective working relationships with Center staff while maintaining the integrity of the role and the reporting relationships required

ESSENTIAL FUNCTIONS:

1. Reviews patient related documentation on a regular basis to assure compliance with Medicare, Medicaid and other required areas of compliance and customer service. Including but limited to:

· Review PCC progress notes and Change in Condition (and follow-up notes) daily to identify issues of concern/requiring follow-up

· Review customer service program feedback and assists with follow-up

· Attends A.M. and clinical meetings to discuss resident changes, offer professional feedback and provide follow-up as needed to concerns identified

2. Reviews all incident reports and investigations. Enters incident report data in tracking system as required and sends updated file to Director/Regional Dir. of Clinical Operations weekly

3. Conducts regular audits of patient records, clinical systems and documentation to assure compliance with policies and procedures and regulatory bodies. Develops a calendar/schedule for audits based on Center’s risk areas unless frequency directed from Director/Regional Director of Clinical Operations. Audits include, but are not limited to:

· Lab audits, ancillary services audits, xray audits

· Advance directives audits, psychotropic medication consent audits

· Allergy audits

· Foley catheter audits

· Coumadin audits

· Sliding scale insulin audits

· Pain audits and narcotic administration audits

· Dialysis and hospice documentation chart audits

· IV/PICC line audits

· Skin/wound care/treatment sheet audit and pressure ulcer prevention audits

· Alarm/device audits, siderail audits, fall prevention implementation audits

· Weight audits, constipation and bowel regimen audits

· E-kit and medication room audits

· Infection control practices/rounds

· Medication storage and/or distribution practices

· Editing accuracy

· Physician visits

4. Provides reports on a regular basis, and as directed or requested, to keep the Director of Clinical Operations and the Sheehan Health Group Management Team informed of the operation and progress of audit efforts and clinical outcomes.

5. Ensures proper reporting of violations or potential violations to necessary management staff within the nursing center and to Director of Clinical Operations.

6. Reviews clinical activities and initiates corrective action/reports to management as needed.

7. In collaboration with Staff Development Coordinator educates personnel in clinical and regulatory issues and Center’s policies, and specialized programs

8. Identifies quality of care issues and takes appropriate action.

9. Rounds resident care areas on a regular basis and identifies opportunities for improvement/”teachable moments”, call bell responsiveness, interactions (staff-to-resident; staff-to-supervisor and supervisor-to-staff) and models desired behavior and interactions

10. Assures that admission and readmission documentation is accurate and fully completed in the appropriate time frame. Educates staff on correct documentation procedures and collaborates with management staff when system issues are identified for company-wide correction and education.

11. In absence of (or in collaboration with) DNS, assists in reviews of residents referred for potential admission

12. In absence of the DNS collaborates with SDC/IC to provides supervision to the nursing department including working with Scheduler to assure proper levels of staff are maintained.

13. Performs other tasks as assigned.

14. Conducts job responsibilities in accordance with the standards set forth by the Management Group, the nursing Center policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.

A Former /current RN - DON who prefers to take a step back from the demands being an RN-DON brings about but, highly enjoys the clerical aspects of clinical reports.

A Former/Current RN- DON with a genuine appreciation for reviewing reports to identify areas needing attention and then seeing the corrections through.

Pay rate: Open to discussion within center's budget, depending on years of experience.
