Position Summary:
  • The IBHH Care Coordinator provides integrated support, advocacy, and coordination of care from admission to post-discharge for clients and families within the IBHH program at LFS. The position facilitates activities to recruit, engage, and retain clients and families in program activities and services.
Job Duties:
  • Provide outreach and engagement activities to facilitate IBHH program enrollment and engagement.
  • Complete comprehensive clinical needs assessment and any required screening for the Health Action Plan (HAP).
  • Develop the HAP by setting member-centered goals and action steps to achieve the goals.
  • Identify possible gaps in services and secure needed supports.
  • Work with staff to link clients to community resources and other clinical providers.
  • Accompany clients to medical and/or other appointments to facilitate their engagement and follow through as needed.
  • Prepare crisis intervention and resiliency plans (wellness recovery action plans (WRAP), relapse prevention plans, etc.).
  • Provide outreach, engagement, and service planning following discharge from higher levels of care.
  • Support the member to live in the setting of their choice.
  • Coordinate with other agencies and health homes.
  • Provide coaching and recovery support on issues including but not limited to self-advocacy, after-care, wellness self-management, medication adherence, smoking cessation, entitlements, daily living activities, etc.
  • Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
  • Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.
  • Establish and maintain positive working relationships with internal and external team members.
  • Assess situations to determine importance, urgency, and risks, and make clear decisions which are timely and in the best interests of the organization.
  • Set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information, and activities.
  • In collaboration with program leadership, continually evaluate processes and workflows related to patient support within the IBHH program and work toward continuous quality improvement.

Education and Experience:
  • Bachelor’s degree in Social Work or other related field required or equivalent work experience.
  • At least two years of knowledge, familiarity, and/or experience in case management services.
  • Demonstrated skills and competencies working with a variety of behavioral health needs.
  • Knowledge of functions and services of community organizations and related human services.
  • High level of organizational skills, communication and interpersonal skills, and attention to detail.
  • Valid Driver’s license, proof of liability auto insurance, and ability to drive for work.
  • Bilingual preferred (fluent in English and another language spoken by our client populations).
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Hybrid work environment (in-office and remote).
  • Laptop and company issued cell phone.
  • Work schedule may require nights and/or weekend hours as needed.
