Region 6 Behavioral Healthcare currently has a position available within the Professional Partner Program (Behavioral Health Care Coordinator) as a “Professional Partner”. The Professional Partner Program is a voluntary care coordination program serving youth and young adults (ages 3-26) experiencing mental health challenges and are at risk of being placed outside the home, school, or community.

The Professional Partner (Behavioral Health Care Coordinator) utilizes a Wraparound approach that brings together a team of people to develop and facilitate a comprehensive service plan and community supports while working to strengthen the connection within the community. The Wraparound approach is strength based, team based, and community based, providing opportunities through this structured and creative process to meet each family’s individualized needs.

This position requires evening hours based on the family’s availability in the community. This is an opportunity to make a meaningful impact on the lives of individuals facing mental health challenges. Requirements:

  • Bachelor’s degree in psychology, social work, or related field
  • Strong knowledge of mental health, substance use disorders, and community resources
  • Strong communication and interpersonal skills to effectively engage with clients, families, and interdisciplinary teams
  • Ability to work independently and manage a caseload of clients with diverse needs
  • Proficiency in electronic medical record systems for documentation purposes
  • Ability to work within a team in the community
  • Ability to navigate complex situations
  • Valid driver’s license and current auto insurance
