POSITION SUMMARY: The TAY Outreach Coordinator/Peer Counselor conducts community outreach with youth, families, schools, and other agencies to locate and assist youth between the ages of 16 and 21 who could benefit from mental health services. This position provides Outreach and Peer Counseling services designed to engage clients and assist them in locating and acquiring community resources. The ideal candidate will display a compassionate nature, have the ability to demonstrate and implement creative thought processes, work independently, and provide exceptional service. Communication, confidentiality and professionalism are essential parts of this position. The ability to work flexible hours including evenings, and managing your work time effectively within 37.5 hour work weeks is essential. RESPONSIBILITIES: The primary duties and responsibilities include, but are not limited to, the following: Develop an outreach plan and implement initiatives to locate, engage, and develop rapport with Transition Age Youth and provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources. Provide services in the community including client’s home, community agencies, court rooms, hospitals, schools, etc. Provide basic assessment of a client’s needs and work collaboratively with the youth to identify the most appropriate client driven solutions. Establish and maintain collaborative working relationships with community partners.Collect data and maintain accurate client files.Assist clients in distress, prevent further escalating issues and stabilize situations. Provide follow-up service.Transport clients if appropriate.Attend all pertinent staff training and meetings including peer support role meetings and ongoing supervision with your direct Supervisor. Complete all required documentation accurately and meet all required deadlinesAct as a role model; self-disclose your mental health history when appropriate.Represent LCSNW and the Outreach Program at various agencies and community meetings.Function in accordance with the LCSNW personnel policies and procedures and administrative practices.Demonstrate respect for and build on the values, preferences, beliefs, culture, and identity of the youth, their family, and their community. SECONDARY FUNCTIONS: Other duties as assigned by the Program Supervisor or Program Manager. Qualifications: Identify as a current or past consumer of mental health services and be in mental health recovery for at least one year. Must be able to work well under pressure, multitask, set priorities, manage time efficiently, and work independently Be able to respect and work cooperatively with a variety of persons from different social, cultural, economic, and educational backgrounds, including coworkers, community partners (social service agencies), the public, and parents of the youths. A clear understanding of, and the ability to demonstrate, professional ethics, boundaries and judgment Professionalism, punctuality, flexibility, and reliability are imperative.Proficient in the use of personal computers and corresponding programs; preferred experience with Microsoft Office, Gmail and Google Calendar or similar web-based and computer programs. Support and uphold the mission, beliefs and values of Lutheran Community Services.Valid driver’s license and auto insurance is required. Must also have transportation.Clearance through the Washington State Patrol (background check) and verification of ability to work around vulnerable populations is required. Clearance of Motor Vehicle Report to meet LCSNW insurance requirements. Education and or/Experience: High School Diploma or GED Lived experience receiving services for mental health and/or substance use.Certificates, Licenses, Registrations: Certified Peer Counselor (CPC) in Washington State or ability to obtain within 90days of hire.Ability to obtain Washington State Agency Affiliated Counselor Registration within90 days of hire.CPR/First Aid (Within 90 Days) Physical Demands/Work Environment: The physical demands and work environment described here are representative of those that must be met/may be encountered by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to traverse throughout the community, talk, and hear. The employee is occasionally required to remain in a stationary position for an extended period of time. The employee is normally engaged in low-activity tasks such as sitting, standing, and walking. The work environment is typical of professional offices; the noise level is usually moderate. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, peripheral vision, and the ability to adjust focus. Coordination tasks are generally performed in a normal office environment and in community-based sites, including churches. Travel to and from determined locations will be required. Travel: In this role you will be required to travel throughout the Columbia Basin. Mileage reimbursement allocated monthly.
