LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.

LTC is seeking dedicated Palliative Care Advanced Practice Providers to join our team as Primary-Care Providers.
