Los Angeles Boys & Girls Clubs Case Manager

This job description is intended to convey information essential to understanding the scope of the Case Manager position and is not intended to be an exhaustive list of skills, efforts, duties, responsibilities or working condition associated with the position.

DEFINITION

Under direct supervision, the job of a Case Manager is to regularly meet with youth participants ages 10-18, helping each create individual goals and track their accomplishments, with the end goal of preventing youth involvement with violence. The Case Manager will also engage family members and caretakers when appropriate. The Case Manager will conduct outreach to the community and internally; build rapport with families and targeted youth; provide case management services in accordance with the program requirements and administration's directions & guidance; establish and maintain records; compile and distribute materials and reports; respond to inquiries from a variety of internal and external sources; and to do other related work as needed.

ESSENTIAL DUTIES

  • Attends site and/or in-service meetings for the purpose of conveying and/or gathering information required to perform functions;
  • Assists with participant outreach by preparing and distributing outreach material; making presentations, phone calls, and scheduling meetings;
  • Maintains a variety of manual and electronic documents/reports, files and records for the purpose of providing up-to-date information and historical reference in accordance with established administrative guidelines and legal requirements. This may include participant information.
  • Assists with assessing participants’ needs, situations, personal strengths and support networks to work with youth and determine appropriate goals;
  • Coordinates with appropriate organizations and services including, but not limited to, social services, physical and mental health services, housing support, employment education and substance use disorder treatment;
  • Researches and refers participants to community resources;
  • Monitors and tracks milestones such as, employment acquisition, and/or educational progress;
  • Performs a variety of general clerical duties including answering phones, completing documents, using calendar, and typing;
  • Responds to inquiries from a variety of internal and external parties (e.g. schools, partners, government agencies, general public, students, etc.) for the purpose of providing information, facilitating communication among parties and/or providing direction;
  • May be assigned other related duties as needed.
  • Meets with youth and families in compliance with program requirements
  • Tracks youth participation in positive activities and provides supports to engage youth in these activities
  • Responsible for all tasks related to holding case management meetings with youth and families

QUALIFICATIONS

Knowledge of:

  • Working with youth in a trauma-informed environment (training available to strengthen these skills);
  • Assessing youth wellness, needs, and abilities and using the knowledge of them to develop plans;
  • Listening to youth/clients’ concerns and providing coaching;
  • Recording youth/clients’ progress including charting referrals, home visit, and other notable interactions;
  • Organization and coordination of specialized and responsible clerical functions;
  • Modern office methods and equipment, including electronic record management and filing systems, computer terminal operational processes, and telephone techniques;
  • correspondence and report writing;
  • English usage, grammar, spelling, and punctuation (potentially other languages based on youth served, bi-lingual applicants strongly encouraged to apply);
  • Basic methods and techniques of organization and planning, following schedule, and creating a case plan for each youth and ensuring requirements for each youth are followed

Ability to:

  • Work effectively with youth, showing compassion and ability to relate to youth/clients with a wide range of needs;
  • Motivate youth to follow established plans;
  • Think critically and problem solve;
  • Flex and change plans if not getting desired results;
  • Manage multiple cases at once;
  • Communicate effectively in oral and written form;
  • Learn, interpret, and apply legal mandates, policies, rules and regulations, and operational
  • procedures;
  • Compose correspondence independently;
  • Establish and maintain comprehensive and accurate files and records, and prepare concise and
  • Drive agency vehicle to transport youth and/or families to community/club events, to scheduled activities, to appointments, etc.

Complete reports as required;

  • Understand and carry out oral and written directions;
  • Apply and explain policies, procedures, rules and regulations;
  • Operate a computer terminal to enter data, maintain records and generate reports;
  • Meet schedules and timelines;
  • Plan and organize work;
  • Work independently with little direction;
  • Understand and carry out oral and written directions;
  • Establish and maintain cooperative working relationships.

PHYSICAL DEMANDS

The physical requirements indicated below are examples of the physical aspects that this position

classification must perform in carrying out essential job functions.

  • Persons performing service in this position classification will exert 20 to 40 pounds of force

frequently to lift, carry, push, pull, or otherwise move objects.

  • Office environment; subject to constant interruptions. This type of work involves sitting most of the time, but may involve walking or standing for extended periods.
  • Perceiving the nature of sound, near and far vision, depth perception, providing oral information, the manual dexterity to operate business related equipment, and handle and work with various materials and objects are important aspects of this job.

EXPERIENCE AND EDUCATION

Any combination of experience and training that would likely provide the required knowledge and skills. A typical way to obtain the required knowledge and skills would be:

Experience:

Two years of increasingly responsible experience working with youth populations, particularly those in areas with high rates of violence.

Education:

  • High School diploma or high school equivalency certificate (GED, HiSET, TASC).
  • Some college
  • BA degree preferred.

Job Type: Full-time

Pay: $45,000.00 - $54,000.00 per year
