Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.
The LPN is responsible for providing end-of-life care and hospice services to patients in their home or community setting. The Licensed Practical Nurse (LPN) is responsible for assisting in planning for the patient's and family's care under the direction of the Registered Nurse Case Manager and Clinical Manager.
