Boston-Area Skilled Nursing Facility seeking a self-motivated individual to join their team and develop a strong referral network. An ideal candidate should ensure effective and successful relationships are maintained with the referral sources and customers resulting in skilled admissions that meet the census goals of the center.
The ideal candidate will:
Ensure effective and successful relationships are maintained with the referral sources and customers resulting in skilled admissions that meet the census goals of the center.
Assess and evaluate potential patients for appropriate admission and facilitate the admission process, working closely with the hospital staff and facility staff.
Seek to build a strong referral network, engage in person-to-person calls and meeting with the referral sources to increase referrals from existing contacts and generate referrals from new sources.
Ensure responses to inquiries are handled properly, timely and with the appropriate follow up.
Manage all aspects of the referral and admission process. Receive and evaluate all referrals to assure clinical appropriateness for the hospital. Work with the hospital to ensure all steps of the admission process are thoroughly completed. Present a completed admission record to the facility.
Maintain excellent working relationship with the medical professionals and other health related facilities and organizations. Promote Company philosophy, mission and objectives within and outside the hospitals. Attend and participate in facility meetings, calls and in-services. Demonstrate honesty and integrity at all times and project a professional image.
Participate in weekend on call rotation.
License/Certification:
Must have a valid driver’s license
RN/LPN/LSW preferred but not required
Experience:
Two years clinical and marketing experience in a multiple service healthcare model with an emphasis on network and business development.
Working knowledge of the various electronic referral platforms.
