Loma Linda University is Hiring a Case Manager-Rehab Intake Near Loma Linda, CA

Job Summary: The Case Manager-Rehab Intake completes complex clinical evaluation for review with Medical Doctor (MD) by gathering necessary medical information on patients being considered for admission to rehabilitation. Travels to other healthcare organizations to evaluate patients for admission. Serves as an admission case manager communicating with third party payors to obtain authorization for acute inpatient rehabilitation. Orients patient and family to rehabilitation programs and process. Works with patient, family and acute case manager to establish discharge plan. Communicates with nursing to arrange staff and bed availability when patient is ready for transfer. Coordinates transfer of patients to rehabilitation when medically cleared and ordered by MD. Obtains financial clearance for patient admission in accordance with admitting department procedures. Develops educational programs for community and third party payors on rehabilitation programs independently. Assists Service Line Director with implementation of marketing plan and provides input in development of marketing budget and maintains expenditures within budget. Builds patient census through growth of new and existing referrals. Facilitates and improves admission access to Rehabilitation services. Assists in the coordination of all public relations functions, mailings and development of promotional materials as assigned. These services may be provided for both the LLUMC and LLUCH hospitals. Performs other duties as needed. Performs other duties as needed.
Education and Experience: Completion of Registered Nursing (RN) program with the minimum of Associate's Degree in Nursing required (Bachelor's Degree preferred). OR graduate of a Rehab Therapy Program (Occupational Therapy (OT)/Physical Therapy (PT)/Speech Pathology (SP)) Master's Degree preferred. Minimum two years of acute hospital adult nursing or two years rehab nursing experience required. Or if OT/PT/SP then four years of Rehab therapy experience required. Minimum two years of case management experience preferred.
Knowledge and Skills: Must have working knowledge of case management and MediCare and MediCal Regulations. Accept and integrate clinical and administrative feedback and guidance. Able to appropriately prioritize, multi-task and complete projects in appropriate time frame. Able to read; write and speak with professional quality; use computer and software programs necessary to the position, e.g., Word, Excel, Power Point, Access; operate and/or troubleshoot basic office equipment required for the position. Able to consistently demonstrate a positive attitude; relate and communicate positively, effectively, and professionally with others; cooperate with management and other associates in achieving organizational goals, clinical program mission, rehabilitation institute mission and hospital mission. Provide leadership; consistently demonstrates constructive interpersonal skills with staff, colleagues, patients, and all consumers, e.g. Explores others differing viewpoints, discuss solutions; be assertive and consistent in enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English (and in Spanish preferred) in person, in writing, and on the telephone; think critically meet documentation standards.; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors on computer screens; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; read documents necessary to position.
Licensures and Certifications: Current California Registered Nurse (RN) License required. OR Valid California licensure as an Occupational Therapist, Physical Therapist or Speech Pathologist required. Basic Life Support (BLS) certification required.
