The Job Coach assists individuals by evaluating their job needs, assisting in preparation for job placement, and instructing them on basic job tasks.

Requirements

Essential Functions

  • Provide on-site job training to people served as well as providing coaching and training on other important life skills to encourage independence.
  • Assist people served in developing resumes, completing employment applications, participating in interviews and other hiring requirements.
  • Review job performance with people served to ensure high quality performance is occurring for the employer.
  • Builds effective relationships with employers that will maximize an employee’s likeliness to be successful; including building natural supports in the workplace.
  • Provides opportunities for participants to be active in the vocational rehabilitation/job search process.
  • Maintains open and professional communication with participants, staff, families, funding sources, employers, and the business community.
  • Completes and maintains all related documentation in accordance with LOGAN standards, including program evaluation data, monthly progress reports, employer contact logs, and any additional required documentation.

Essential Functions -- Assessment

  • Completes referral and admissions process for individuals referred.
  • Completes a range of assessment options and leads the Discovery process with the individual served.
  • Participate in support team meetings to review assessments completed, provide recommendations for future vocational planning, establish and review goals, as well as complete necessary documentation and formal reporting.

Essential Functions – Quality/Service Assurance

  • Ensure the implementation of vocational and related supports for individuals assigned.
  • Promote health and safety of all staff and persons served through adherence to all LOGAN policies and procedures.
  • Maintain all appropriate information in a confidential and professional manner, and in accordance with regulatory guidelines.
  • Model appropriate behavior in interaction with persons served, staff, families, community resources and other audiences/customers.
  • Actively participate in personal professional development through use of the Employee Evaluation Process, training opportunities and interaction with other staff and professionals.
  • Ensure that all communication takes place in a professional manner, reflecting both dignity and respect for persons served.

General Responsibilities

  • Expected to uphold the mission, vision and values of LOGAN through attitude and actions of the individual.
  • Expected to serve as a role model for persons served.
  • Expected to serve as an advocate and to protect the human rights of persons served.
  • Ensure that all communication takes place in a professional manner, reflecting both respect for persons served and the LOGAN image.
  • Seek professional growth by keeping current on new philosophy, research, technology, strategies, etc. Keep informed of industry trends and best practices including participation in the Indiana Association of Rehabilitation Facilities (INARF) professional interest sessions.
  • Achieve and adhere to CARF accreditation standards.
  • Develop and monitor the department budget working toward financial targets. Ensure proper documentation of services and submittal of all billing information on a timely basis.
  • Other duties as assigned by supervisor.

QUALIFICATIONS

Education

  • High school diploma or GED

Experience

  • Two years of experience in social service environment. Experience in job analysis and work site training, as well as an understanding of supported employment.

Knowledge, Skills, and Abilities

  • Good interpersonal and communication skills necessary to build relationships with community organizations, clients, funding source staff and LOGAN staff.
  • Ability to accurately interpret and consistently communicate policies and procedures.
  • Must have and/or develop working knowledge and proficiency in Person Centered Planning and Positive Behavior Supports.
  • Oral/written communication skills to clearly and effectively express ideas.
  • Ability to plan and organize work activities/processes, use discretion and sound judgment and demonstrate initiative to ensure goals are achieved in a timely manner.
  • Must provide own transportation, have valid driver’s license and driving record must meet insurability requirements.
  • Must maintain and provide documentation of auto insurance at limits set by LOGAN.
  • Must provide record of vehicle registration.
  • Must have ability to lift/move individuals or materials weighing up to 50 lbs. periodically.
  • Must maintain CPR and First Aid certification, and must stay current on all other training as defined by state, accreditation body or organization guidelines.
  • While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts, and fumes or airborne particles. The noise level in the work environment is usually quiet to moderate.

Fulfilling the responsibilities of this job may result in contact with body fluids. LOGAN will provide training and offer Hepatitis B vaccination at no cost to the employee.
