Little Rivers Health Care, a FQHC located in Wells River, Vermont seeks a Behavioral Health Director. Our multi-disciplinary care team is a unique model of fully integrated medical and behavioral health care with licensed therapists and care managers. Our team consists of family medicine physicians, nurse practitioners, licensed behavioral health providers, psychiatric mental health nurse practitioners, and care coordinators. We serve a rural low-income community, with an agricultural farmworker population. The health center provides extensive community health and prevention programs, a robust MAT program, and school-based services. Competitive salary and benefit package, reasonable on-call. BEHAVIORAL HEALTH DIRECTOR POSITION SUMMARY

The Behavioral Health Director contributes to the successful achievement of the Little Rivers Health Care (LRHC) mission and strategic initiatives by providing leadership, oversight, evaluation and direction of LRHC Behavioral Health operations. With a focus on patient care, the Behavioral Health Director assures adequate provider productivity within a quality assurance process. Responsible for overseeing health center activities related to the delivery of behavioral health services to the community. Serves as the primary liaison between LRHC departments and community stakeholders. The Behavioral Health Director will provide care to LRHC patients, evaluating medical and psychosocial conditions; developing and implementing treatment plans; monitoring and evaluating treatment results.

SUPERVISION

Supervisor: Chief Medical Officer

Direct Supervision: Behavioral Health providers

ESSENTIAL JOB FUNCTIONS

  • Provides behavioral health services to patients in conformity with clinic protocols and professional code of ethics. Also, ensures compliance with federal, state, and local standards and contributes to the efficiency of health center operations.
    • Performs oversight of behavioral health services provided to patients by the Behavioral Health providers of LRHC.
      • Monitors and ensures that the performance quality by Behavioral Health providers meets federal, state and local standards.
      • Reviews and resolves patient complaints directly related to behavioral health care.
      • Creates, assists, maintains, modifies and implements LRHC policies and procedures in compliance with current accepted professional standards.
      • Ensures all requirements of the electronic medical record are met, including but not limited to, documentation of patient encounters, meaningful use, patient self-management goals and review of patient laboratory data.
      • Ensures that professional persons and facilities used for patient referrals meet the applicable standards of care.
      • Knowledgeable in HRSA guidelines and ensures that LRHC maintains compliance with all applicable regulations and requirements.
      • Conducts Behavioral Health provider staff meetings and ensures meeting documentation is maintained.
      • Improves Behavioral Health provider competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, crisis intervention, and related issues.
  • Quality Management oversight
  • Directs the behavioral health quality assessment and quality assurance/improvement program in compliance with applicable federal, state and local regulations, and ensures meetings are appropriately documented.
  • Establishes and maintains a patient care audit process that ensures the quality of care provided meets acceptable standards.
  • Coordinates the internal peer review process and patient chart review.

3. Acts as behavioral health liaison to the community and provides support to the Chief Medical Officer:

  • Serves as the behavioral health professional representative to the healthcare community and other organizations as appropriate.
  • Develops positive relationships with local behavioral health groups, behavioral health societies and consumer groups.
  • Evaluates patients by interviewing patients, family, and other persons; conducting physical examinations; observing behaviors; reviewing medical history and related documents; selecting, administering, and interpreting psychological tests; ordering laboratory tests and evaluating results.
  • Develops treatment plans by determining nature and extent of cognitive, emotional, developmental, social, and behavioral disorders; establishing treatment goals and methodologies.
  • Treats patients by utilizing psychotherapeutic methods and medications; discussing progress toward goals with patients; providing instructions; monitoring effect of medications; supervising staff provision of services.
  • Maintains interdisciplinary treatment by reviewing treatment plans and progress; consulting and collaborating with primary care physicians, mental health therapists, and other health care providers.
  • Assures quality and safe service for patients and staff by enforcing policies, procedures, standards, rules, and legal/regulatory requirements; participating in utilization reviews; remaining available for emergency consultations.
  • Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries.
  • Updates job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

4. Assists CMO and serves as team lead to ensure that federal, state, and local standards are met by LRHC as they relate to the performance of behavioral health services

  • Assists senior management in meeting all internal and external performance and reporting requirements.
  • Assists in setting and managing organizational performance goals.
  • Recruits professional behavioral health staff, drafts contracts and onboards new hires.
  • Serves as a role model and is accountable and responsible for ensuring optimal performance of clinical operations; quality of care, customer service and productivity.

QUALIFICATIONS:

  • MD, DO, PMHNP, LICSW, or LCMHC
  • Maintains current unrestricted professional license.
  • Board certified or board eligible, if applicable
  • Maintains Current Cardiopulmonary Resuscitation Certificate (AHA- BLS/Provider)
  • Complies with required license Continuing Education requirements.

SPECIFIC SKILLS:

  • Provides complex information to the organization.
  • Articulates organizational vision and implements organizational strategic initiatives.
  • Possesses a high level of verbal and written communication skills including the ability to respond accordingly to patient and staff needs.
  • At a minimum the candidate must have strong computer skills including the ability to prepare internal and external correspondence and reports and be able to summarize data from the electronic medical records system.

EXPERIENCE:

  • A minimum of three (3) years’ experience working in behavioral health, FQHC experience is preferred.
  • Past supervisory/leadership experience
  • Public Health or Community/Rural Health Center experience preferred.

WORKING CONDITIONS AND PHYSICAL DEMANDS:

  • General office environment
  • Frequent time spent in LRHC patient care sites
  • Occasional travel
