Summary Job Description:

The Coordinator, Behavioral Health Services is responsible for the oversight, development and implementation of person-centered behavioral supports and services to individuals residing in Little Flower’s OPWDD residential programs. Responsibilities include but are not limited to:

  • Conducting the Functional Behavioral Analysis
  • Baselining identified and targeted behaviors
  • The development of Behavior Support Plans, analyzing data collection, counseling individuals
  • Supervision of Behavioral Intervention Team for the Division.
  • Following up with Mental and Emotional Health Crisis’
  • Collaborating with the Psychology Team on all outcomes
  • Preparing data for the division with behavioral health metrics.
  • The completion of all required documentation as per OPWDD’s regulations and Little Flower’s policies and procedures.

The Coordinator, Behavioral Health Services will take a strong lead in communicating with and advocating on behalf of the individuals/agency to doctors, practitioners, clinicians, family members, etc., in the community towards diagnosing conditions, developing plans of care, and evaluating responses. The Incumbent, in conjunction with the residential team, will provide oversight of psychiatric medications, including obtaining required consents and providing written reports to the treating psychiatrist/clinician. The incumbent will be responsible to provide clinical trainings and support to direct support professionals on behavior plans, appropriate interventions, clinical diagnosis, etc. The Incumbent will serve as the Behavioral Health Services leader in the residential programs.

Essential Functions:

  • Serve as the clinical role model and leader in Little Flower residential
  • Manage the Behavioral Intervention Specialist teams in Queens, Brooklyn, and Long Island.
  • Go to all the Residential Programs to meet with the BIS staff and Residential Team members to conduct observations of behavioral intervention techniques and provide in-services.
  • Draft in-services related to Behavioral Intervention techniques and ensure all staff members of that program are captured.
  • Conduct monthly supervision for all direct reports and submit supervision reports to Administrator and Senior Vice President for review by 5th of each month.
  • Be present for all surveys at the residences in Queens and Long Island.
  • Actively participate and present at Human Rights Committee (HRC) meetings, Life Plan (LP) and LP review meetings, SAP and SAP review meetings, staff meetings, trainings, Interdisciplinary Team (IDT) meetings and other forums as requested by
  • Advocate to and interact with psychiatric, psychotherapy and psycho-sexual services when
  • Conduct internal audits and submit findings to Administrator monthly by preparing an audit log.
  • Document all progress notes in the Electronic Health Record system LF uses.
  • upload consents, data sheets, etc. into the Electronic Health Record system.
  • Hold IDT Meetings for the Residences each month, take minutes, and distribute the minutes to all leadership staff members via email. Must maintain minutes in a binder for audit review.
  • Obtain required consents from families for Human Rights committee for medical treatment (including medication), behavior modification plans, restrictive
  • Conduct internal audits of clinical policies, procedures, and systems to ensure regulatory compliance as per OPWDD and all Little Flower policies, procedures, and best
  • Consult with doctors, clinicians, to ensure optimal clinical treatments are provided to people being supported.
  • Ensure equity for all residents and properly advocate for their residential rights.
  • Collaborate with each team member to plan best course of treatment for maximum clinical health of people being supported.
  • Ensure completion of Functional Behavioral Assessments and development of individualized behavior support plans and staff interventions as per OPWDD
  • Ensures Behavioral Plans are accurate and Person-Centered as they are deemed legal and clinical documents.
  • Behavioral Plans must be uploaded into EHR system LF utilizes and placed in a binder in the Manager’s office for survey readiness.
  • Provides monthly data on resident’s progress, such as hospitalization due to behaviors (charts, graphs, spreadsheets, etc.). This is to be submitted to the Administrator and SVP of the programs each month.
  • Implements and monitors effective behavioral data tracking systems for LF and OPWDD.
  • Escalate counseling services in the event of a crisis and coordinate with the CCO and report back to the Administrator of next steps.
  • Provide training to staff on behavior treatment plans, SCIP Trainings, and on topics/plans related to behavioral health, especially during any changes in the plan or changed behaviors such as elopement or new triggers.
  • Ensure all incident management documents are submitted within 24 hours as related to BIS workflow, plans, statements, etc.
  • Engage individuals in individual and/or group treatment as needed (active treatment).
  • Enter the Active Treatment Plan into the Electronic Health Record LF utilizes.
  • Coordinate psychological care and behavioral programs with families, medical providers, residential settings, or other relevant stakeholders in the individual’s service
  • Track hospitalizations due to behaviors in a spreadsheet and enter into the Electronic Health Record. Must also present this data monthly on PowerPoint to the Senior Vice President.
  • Provide crisis intervention, as well as instruct staff in appropriate techniques for addressing challenging behaviors. These steps must be documented in the EHR system.
  • Attend interdisciplinary team meetings, as appropriate, providing input particularly on individuals with identified or emerging behavioral
  • Provide training and in services for BISs, Managers and DSPs and the documents and staff roster must be placed in the binder in the Manager’s office for survey readiness.
  • Participate in staff meetings and attend other meetings, committee/specialty, and provide feedback to staff as requested by the Administrator.
  • Ensure health, safety & welfare of individuals we serve and is an active participant in ensuring the resident is thriving under our care.
  • Travel to various locations throughout NYC and Long Island is Home Base office is in Queens, NY. This is a client facing position.
  • Become a Certified SCIP Instructor within first year of employment to train other staff members.
  • Other duties as assigned by the Administrator or SVP.

Qualifications & Work Experience:

  • Master’s degree in related field preferred, Bachelor's Degree Required
  • Must be CPR/AED certified and it must be current and renewed every two years.
  • Must obtain SCIP Certification within 90 days of employment as part of probationary review.
  • Valid NYS driver's license is required to conduct site visits to multiple sites/Residences
  • 1-3 years of OPWDD clinical experience
  • Maintains a cooperative relationship among teams by communicating information, responding to requests, building rapport & participating in team problem-solving methods
  • In-depth knowledge of OPWDD services, regulations and best practices required
  • Bi-lingual preferred
  • Excellent communication, verbal, written and people skills are required
  • Must be computer literate and have working knowledge of PowerPoint, Excel, Word, Google Docs, and Access.

Travel Requirements:

This position requires travel throughout Queens, Brooklyn, and Long Island. It may also require periodic travel to trainings/meetings in Manhattan. A valid NYS driver’s license is required.
