Position Overview:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our dynamic home health care team. The ideal candidate will have a strong background in nursing, with a passion for providing exceptional patient care in a home setting. This position involves working closely with patients of various ages and health conditions, administering medications, wound care, monitoring vital signs, and providing education and support to patients and their families.

Key Responsibilities:

• Conduct initial patient assessments and develop individualized care plans.
• Administer medications, perform wound care, and monitor patient health status.
• Educate patients and their families on disease management, medication, and home health care procedures.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
• Maintain accurate and timely documentation of patient care and progress through our Electronic Medical Record (EMR) system.
• Ensure compliance with all federal, state, and agency regulations regarding home health care.

Qualifications:

• Valid RN license in the state of practice.
• Minimum of 1 year of nursing experience, preferably in home health care or related field.
• Proficiency with Electronic Medical Record (EMR) systems and technology used in home health care.
• Exceptional clinical skills, including the ability to perform comprehensive patient assessments and administer medications.
• Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare team members.
• Ability to work independently in a home setting, demonstrating critical thinking and problem-solving skills.
• Current CPR certification.
• Must have a valid driver's license and reliable transportation.
