Home Health Physical Therapist Responsibilities

• Confer with referring physicians regarding the treatment of patients
• Visit clients for in-home initial and follow-up visits
• Develop and execute care plans for your patients
• Instructs patients and families in transfer techniques, exercise programs and activities
• Document the course of patient care including progress made, continuing need for treatment and response to treatment
• In-home visits are M-F during normal hours

Home Health Physical Therapist Background

• Prefer 1 years of Home Health experience but open to training
• Pennsylvania PT License
• Clean driving record with reliable transportation
