Brief Job Description:
Seeking flexible and energetic person to provide case management services for individuals with intellectual and developmental disabilities in Warren and surrounding counties. Primary responsibilities include making home visits, completing assessments, creating/updating plans, and monitoring outcomes.

• Flexible work schedule Monday through Friday, which could include working from home.

Qualifications include:
