Title: ERA Supported Housing Case ManagerWorksite: Service Center/CommunityCounty: WarrenHours Status: Full-TimePay Range: $15.87 per hour 

Brief Job Description:Provide outreach services to individuals/families who are experiencing homelessness. This position involves direct services to clients and collaboration with community partners to assist the client in reducing barriers to achieving long term stable housing.

Duties include, but are not limited to, assessing individuals’ strengths and needs; assisting with housing search and placement; assisting individuals with accessing community resources; providing ongoing support to individuals while participating in the ERA program; building and maintaining professional relationships with landlords and community partners. Position requires travel within 10-County service area.

• Outreaching to individuals in the community
• Assisting with completing intake paperwork for HMIS
• Entering intake date into HMIS
• Collaboration with community partners
• Linking with community resources
• Housing search and placement
• Coordinating with landlords/property management companies for participants to obtain housing
• Mediating issues with landlords and tenants
• Ongoing Case Management services with participants while still enrolled in the Rapid Rehousing program
• Attend weekly team meetings
• Attend monthly Coordinated Entry meetings
• Completes all documentation as required by the agency and program.
• Performs other relatedwork as required and assigned.

QUALIFICATIONS:

• Minimum of a Bachelor’s degree preferably in Psychology, Social Work, Sociology or Human Services
• Genuine interest and experience in working with individuals experiencing homelessness
• Valid Kentucky Driver’s license, insurance and reliable transportation
