LifePath, Supportive Services for Older Adults, a local not-for-profit agency has an immediate opening for a Social Worker.

Summary: The Social Worker is responsible for: (1) assuring that LifePath meets its obligations of providing case management, educational, counseling and support activities for seniors and (2) developing and maintaining supportive relationships with community partners to assure a steady stream of referrals and partnership opportunities for ALL programs personally or through subordinates.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

· Oversees all case management, service coordination for Albany Housing Authority at Westview Homes, and counseling activities by providing direct services to seniors

· Develops and maintains a comprehensive network of education, training, mental health, substance abuse and other supportive services for seniors.

· Identify needs, define clear and measurable goals to address those needs and connect with resources.

· Assists in formation of and coordinate monthly meetings of a Resident’s Council meeting.

· Works closely with the Director of Program and Community Outreach to assure that all contractual obligations of the ROSS grant (a public housing resident program funded through HUD) are met and that all output and outcome data is collected and submitted in a timely manner

· Submits all required monthly, quarterly and annual documents/reports to the Director of Program and Community Outreach in a timely manner

· Recruits and supervises volunteers and social work interns

· Identifies program partners for program expansion and synergy

· Speaks highly of the organization on and off duty

· Provides staff support to agency-wide initiatives or projects as requested by Executive Director

· Other duties as assigned

Education and/or Experience: Bachelor Degree in Social Work required; Master's degree (M. A.) preferred; three or more years doing case management in Albany County required; experience working with elderly desirable; experience working with/ managing volunteers/interns. Experience with computers and online programs for communication and recordkeeping.

Pay: $55,000.00 - $60,000.00 per year
