The Massry Residence, the Assisted Living residence of Daughters of Sarah Community for Seniors, is currently seeking applicants for a Part Time Social Worker, this is a temporary position from June through September.

Successful candidate will be responsible for implementing The Massry Residence’s policies and procedures for case management including but not limited to an initial and ongoing assessment of a resident’s strengths and needs. The Social Worker provides oversight and coordination of the Individualized Service Plan (ISP) and identified services; monitoring the receipt of such services and evaluation of the effectiveness in improving resident functioning and quality of life; documenting the activities and their outcomes.

Other basic responsibilities will include:

• Meeting with prospective residents, their representatives, and legal representatives to discuss matters described in the residency agreement.
• Strengthening the informal relationships among residents.
• Developing a formal mechanism of communication between the case manager and facility staff who serve the resident.
• Maintaining complete and accurate records for each resident.
• Identifying adjustment problems and assist residents to resolve adjustment problems if needed.
• Assisting each resident to maintain family and community ties and to develop new ones.
• Encouraging resident participation in community activities.
• Acts as a liaison for residents and families with outside agencies or other residents.
• Assists in facilitating any resident’s move from the Massry.

Requirements:

• Requires a Bachelor or Master’s Degree in Social Work from an accredited school of Social Work;
• One year of supervised social work experience in a health care setting, working directly with individuals is preferred;
• Working knowledge of New York State Department of Health regulations preferred.
