Life Empowerment Services is Hiring a Social Service Coordinator Near Marietta, GA

Position Title: Social Service Coordinator

Reports to: Director of Clinical Operations

Job Status: Full-Time Position

Pay: Range starts at $34,000 per year plus bonus pay, mileage reimbursement, and benefits
FLSA Status: Exempt

POSITION SUMMARY
The Social Service Coordinator will coordinate and oversee the direct service processes of the agency, to include providing support to the social service team. This position encompasses administrative responsibilities and direct services (Behavior Aide, Transport, and Drug Screen services).

EDUCATION

  • Bachelor’s degree in a Human Services-related field or high school diploma and 5 years’ experience in the human services field
  • Human Services degrees include: Psychology, Sociology, Counseling

(religious counseling is not acceptable), Social Work, Child Development,Community Mental Health, Human Services, Criminal Justice (only withJuvenile experience/DJJ), or Education (experience teaching K-12 grade)

  • Minimum of one year of working within the human services field, providing support services (supervised visitations, parent aide services, drug screens, etc.)
  • Experience providing a combination of direct services and administrative support in a clinical environmentESSENTIAL DUTIES AND RESPONSIBILITIES

Responsibilities will include but are not limited to:

  • Coordinate social services and provide support to the clinical services team, including assigning cases, requesting services, and reviewing case notes
  • Monitor social service referrals to ensure that services are completed efficiently and timely
  • Address any concerns or complaints concerning the social service team to ensure that policies and procedures are followed
  • Recommend corrective action for social service team members as appropriate and create performance improvement plans when needed
  • Identify and recommend solutions to clinical service team and agency to resolve issues or problems
  • Ensure that all support team members have access to support service resources and screening tools needed to complete referred services
  • Conduct periodic support meetings with social services team
  • Build relationship and collaborate with case managers, contractors, and community partners to ensure effective treatment and case collaboration
  • Assist with interviews and also conduct orientation and training on clinical policies and procedures
  • Create and facilitate training to support the agency service goals
  • Perform social services to include Behavior Aide (parenting classes, supervised visitations, etc.), Transport, and Drug Screen services
  • Document case notes in the online electronic health record system (eCase Notes)
  • Maintain excellent communication and effective working relationships with other agency staff
  • Manage and implement agency processes to ensure a standard of care is met
  • Attend agency meetings and trainings pertinent to the role and relay relevant information to social service team
  • Embrace the vision and mission of the agency
  • Performing other work-related duties as assigned

COMPETENCIES

  • Knowledge of electronic health record and practice management systems. eCase Notes and/or ARGO experiences a plus

  • Efficient with strong attention to detail
  • Self-starter with strong customer support orientation (for internal/external customers) and demonstrated professional demeanor
  • Ability to pass all vital background checks/screenings

WORK ENVIRONMENT

  • In office position with some community engagement
  • Based on a 40-hour work week with at least 1 evening day
  • Some weekend work may be required
  • Interaction with a diverse population including team members, providers, clients, etc.

The noise level in the work environment is usually low to moderate.
