Lenape Valley Foundation is Hiring a Case Manager | Full Time Near DOYLESTOWN, PA

Join our Case Management Team as an Administrative Case Manager. This is a full-time role based in beautiful Doylestown, PA. In this critical role, you will support all Lenape Valley Foundation (LVF) services and programs in helping clients to access critical community services.

What You Offer: 

  • Bachelor’s degree in Human Services, Psychology, Social Work, Sociology, or related field PREFERRED
    • HS Diploma with 12 semester credit hours in Sociology, Social Welfare, Psychology, or other Social Science AND 2 years’ experience in direct contact with individuals with behavioral health issues accepted.
  • Strong computer skills with proficiency in Microsoft Office, Excel, and other software programs
    • Proficiency in Electronic Health Records (EHR preferred)
  • Ability to interact with clients who exhibit a broad range of behavioral health issues
  • Ability to manage processes, provide organization, analyze information, exhibit problem solving skills and attend to follow up tasks.
  • Ability to perform the physical requirement of the job description, with or without accommodation

What You’ll Do: 

  • Provide short-term case management for individuals registered or seeking services within Lenape Valley Foundation.
  • Help clients access critical community supports such as: Health Insurance, Free Prescriptions, Housing, and more.
  • Receive referrals for services and provide immediate follow-up.
  • Document all work within an electronic health system
