Job Description

The Bucks County Drug & Alcohol Commission, Inc. is seeking a full-time Drug and Alcohol Case Management Specialist with demonstrated leadership abilities, case management experience and project management skills. Superior organizational and communication skills are required. The successful candidate will be able to work independently while functioning as part of a team.

The position is located in the Bucks County Administration Building in Doylestown, Pennsylvania, and will require travel throughout Bucks County, as well as some travel to other counties. The position requires an on-site presence (in office or in the community) five (5) days a week. Occasional weekend and evening availability to attend community events is also required.

Job Responsibilities:

Case Management:

*Work with individuals awaiting or engaged in drug and alcohol treatment, as well as those who are scheduled to conclude treatment, to ensure participation and connection to appropriate services, including after care.

*Identify individuals with recurrences of substance use including those with complex/ co-occurring needs and connect the individuals to services and other resources as needed.

*Perform other Case Management and accountability duties to include:

  • Serve as the primary point of contact for assigned cases.
  • Advocate for the individual and provide support and encouragement while in case management.
  • Work with all relevant parties including, but not limited to, the justice system, children and youth services, treatment providers and other service systems to ensure appropriate and coordinated services are delivered.
  • Monitor the individual’s level of engagement, participation in and progress with the established treatment plan and associated services, encouraging follow through, as needed.
  • Collect and report data as needed to comply with Agency, state and/or federal requirements.
  • Communicate provider and system issues/concerns to appropriate departmental staff for review and resolution
  • Represent Agency at community meetings/community events, as requested.
  • Provide education and training to our County and community partners on addiction, substance use disorder, prevention, treatment, recovery resources, harm reduction, use of Narcan (Nalexone) and other identified topics of interest.
  • Participate along with other staff members in the Agency’s annual monitoring of service providers' deliverables and complete reports utilizing established quality assurance assessment tools..

Qualifications

Degree in a related field (Drug & Alcohol Counseling, Social Work, Marriage and Family Therapy), with a minimum of 3 years of experience, preferably working with individuals dealing with substance use disorders.

Pennsylvania licensure and/or certification (LPC, LCSW, LMFT, CADC, CAADC) is a plus.
