Legado Management LLC is Hiring a RN for Legacy Home Health Agency, Inc. Near Corpus Christi, TX

The Registered Nurse (RN) is responsible for administering skilled nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care. This is a clinical position reporting to the Director of Nursing (DON), providing skill nursing services to patients of Legacy Home Health Agency.

Essential Functions:

  • Initial and ongoing comprehensive assessments of home health patient’s needs, including OASIS assessments at appropriate time points.
  • Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.
  • Provide those services and/or treatments requiring substantial and specialized nursing skill. Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA)
  • Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service. Monitor assigned cases to ensure compliance with requirements of third party payor.
  • Provide effective communication/counseling to patient/family, team members, and other health care professionals in meeting nursing and other needs.
  • Complete appropriate documentation in a timely manner.
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned.

Job Qualifications:

  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Licensure: Current Texas State license as a Registered Nurse, current Texas Drivers License
  • Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, home health preferred.
  • Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. If visits are required, Proof of current CPR, negative TB screen and Hepatitis profile.
  • Transportation: Reliable transportation. Valid and current auto liability insurance.
