We are seeking motivated individuals to coordinate the start of care for new patient/clients for the in-home provider care program (also known as PAS or PHC services) atLegacy Home Health Agency, Inc.by providing a channel through which patients/clients or caregivers can seek solutions to problems, concerns and/or unmet needs.

Responsibilities will include:

• Answering high volume incoming calls regarding patient/client services.
• Processing patient information in agency database.
• Communicating with state caseworkers and insurances to coordinate services.
• Follow up on client status as needed.
• Conduct home visits to initiate services.
• Travel to physicians offices to acquire documentation if needed.
• Perform other duties as assigned.

Job Qualifications: 

• Customer service experience required.
• Work experience in PAS/PHC Coordination or in a healthcare related fieldpreferred.
• Bilingual in Spanish required.
