Learning Services , a Collage Rehabilitation Partners is looking for a Rehabilitation Case Manager Assistant to join our outstanding team in our Roswell, GA location. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

Responsibilities

This position works closely with the program’s Rehabilitation Case Manager, clinical team, and administrative personnel to meet the documentation, medical records requirements and billing needs of the company.

Essential Duties & Functions

  • Monitors and maintains the collection of medical record forms, documentation and data for the client chart.
  • Reviews and updates medical records, and client profiles regularly; ensures all information is accurate.
  • Maintains clients’ charts through routine filing and chart management.
  • Review and purges all records according to prescribed time table as determined by the clinical team.
  • Ensures proper storage of all charts and other clinical documentation within the programs.
  • Review and audit chart for necessary documentation and proper order of material.
  • Completes pre-audits and ensures chart readiness for outside auditors in collaboration with the Rehabilitation Case Manager and administrative personnel.
  • Ensures all forms in the client charts are uniform and conform to accreditation standards and company policies and procedures.
  • Maintains listing of current clients in applicable programs and clients admitted/discharged; reviews and updates list as directed.
  • Maintains state licensing forms and ensures supporting documentation is completed upon each client admission, and annually as needed.
  • In collaboration with the clinical team, monitors all outstanding session and log notes to ensure their completion; prepares same for billing; notified the program management team if documentation is missing.
  • Works with the program’s clinical team to ensure completeness and accuracy of the data.
  • Develops and maintain trackers in the client specific program.
  • Assists in monitoring and facilitates timely completion of clinical assessments and documentation as required.
  • Attends client-physician appointments upon request or as needed in emergency situations.
  • Contributes to the implementation of necessary policies and procedures related to documentation for accreditation, licensure and compliance requirements, i.e., HIPAA, CARF, and in conjunction with state licensing requirements.
  • Meets regularly with clients to provide support and guidance
  • Maintains regular contact with family members/support persons/ funders/ and other stakeholders as needed under the guidance of the program case manager.

Qualifications

Education & Experience Requirements

  • A high school diploma and three to five years of healthcare experience with strong office/ administrative skills is required; an associate’s or bachelor’s degree is preferred.
  • Proficient data base entry and upkeep is essential.
  • Proficient word processing skills are essential.
  • Excellent planning and time management skills are required.
  • The ability to communicate with professionals and coworkers is necessary.
  • A valid driver's license and responsible driving behaviors must be maintained.
