The7 day on 7 days off RN on call is a full-time employee who establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
The Nurse Case Manager is a full-time employee who establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. The Nurse Case Manager directs the care of his/her patient caseload, communicates with other team members, families, and ALF/SNF staff.
The Nurse Case Manager coordinates the total care of the patient, including communication with the IDT members, families, Attending MDs, and facility wellness coordinators. Paperwork is to be completed and turned into the office in a timely manner. Responsible for covering calls at least 7 days per calendar month. Must have dependable transportation.
Work is performed in a standard office setting which requires prolonged periods of sitting and working in front of a computer screen. Visual concentration is required. Must be able to multi-task and know how to prioritize. Work requires long periods of time on the phone. It is minimal, or no travel required. Must be able to lift up to 20 pounds.
