On-Call Hours: Mon - Thur 5pm - 8am, Fri 5pm - Monday 8am

Job Summary:

The7 day on 7 days off RN on call is a full-time employee who establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. 

The Nurse Case Manager is a full-time employee who establishes, implements, and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. The Nurse Case Manager directs the care of his/her patient caseload, communicates with other team members, families, and ALF/SNF staff.

The Nurse Case Manager coordinates the total care of the patient, including communication with the IDT members, families, Attending MDs, and facility wellness coordinators. Paperwork is to be completed and turned into the office in a timely manner. Responsible for covering calls at least 7 days per calendar month. Must have dependable transportation.

Qualifications:

1. Graduate of an approved school of nursing.
2. Current unrestricted registration in the state.
3. A current nursing License in the state, with at least 3-year experience in home health/hospice.
4. Must understand the physical psychosocial and spiritual concerns of the terminal patient and the effects of the prognosis on the family.
5. Must demonstrate good interpersonal skills to patients, families, team members, and other health care professionals.
6.  7Days On and 7 Days Off. 

Responsibilities:

1. Respond to emergency telephone requests for patient care, questions or concerns, and medical emergencies.
2. Turn on the cell phone at all times or notify the answering service of how you can be reached.
3. Patient visits after hours/weekends are for emergency purposes. A phone consultation will be used to provide instructions to family/caregivers to help solve problems. After advice is given, the nurse will follow up within a reasonable amount of time to reassess the situation. If there is no improvement and/or the situation is unstable, a visit will be offered and/or made by the nurse.
4. Calls will be responded to within 10 minutes of receiving the signal on the pager or answering service.
5. Documents all contacts on Brightree.

Work Environment:

Work is performed in a standard office setting which requires prolonged periods of sitting and working in front of a computer screen. Visual concentration is required. Must be able to multi-task and know how to prioritize. Work requires long periods of time on the phone. It is minimal, or no travel required. Must be able to lift up to 20 pounds.
