This Victory Parkway Coordinator is responsible for the daily operations of the assigned sites. Supervises and coordinates activities of the Lead Direct Support Professional and Direct Support Professional (DSP). Provides either directly or indirectly the provision and coordination of a variety of supports for people receiving services that will ensure their health and safety.
Essential Functions of this position:
People Leadership
  • Participate in the interviewing process and hiring of DSPs as assigned.
  • Provide training to new employees such as orientation, on-the-job training, health and safety practices, policies, procedures and technologies.
  • Educate DSPs on Individual Service Plans, individual specific training and the proper documentation of progress.
  • Provide guidance to DSPs through individual supervision meetings and team meetings.
  • Monitor, provide feedback and conduct employee performance reviews.
  • Facilitate achievement of employees through participation in internal and external training opportunities.
  • Model professional appearance, conduct and communication.
  • Victory Parkway- provide oversite to all campus sites when present including monitoring DSP performance, providing direction, and troubleshooting problems that occur.
Operations and Compliance Leadership
  • Provide day-to-day leadership and management that mirrors the adopted mission, vision and core values.
  • Ensure compliance with applicable Ohio Department of Developmental Disabilities Administrative Code and Revised Code. Responsible for having a detailed knowledge of LADD policies and procedures, licensure rules if applicable, and ensuring implementation. Has a working knowledge of other applicable regulatory entities as they relate to the management of the assigned department (e.g., HCDDS, HUD, CARF, CMS).
  • Reconcile person served financial records as required by rule, ISP and policies and procedures. Licensed homes- ensure compliance with room and board expenditures.
  • Prepare for and participate in DODD waiver, HCDDS reviews and licensure reviews for licensed homes.
  • Oversee and ensure that an appropriate staffing pattern is maintained for the supports required in the ISP for each person.
  • Work with Scheduler and Lead DSP to ensure shift coverage, process requests for leave and oversee productivity, and scheduling of employees during shifts.
  • Assign responsibilities to DSPs to ensure the effective and efficient delivery of services identified in the ISP for which LADD is responsible.
  • Oversee all required documentation, record keeping, financial and benefit management as required by regulatory agencies and LADD policies and procedures and ensure filed in a timely manner.
  • Respond to all emergencies and emergency calls of persons served or employees and take appropriate action.
  • Fill in for absent employees if needed
Ensure proper Individual Service Plan implementation
  • Represent LADD in the Individual Service Plan (ISP) for each person receiving services on assigned caseload and review completed plans for health/safety needs, accuracy and consistency with LADD’s documentation system.
  • Assist with reviewing referral information and making recommendations regarding enrollment and LADD’s ability to meet the needs of the person referred.
  • Assist persons served with moves and transitions between programs.
  • Submit, as necessary, ISP revisions when the plan does not reflect actual service needs for the person served.
  • Write necessary ISP documents, implement outcomes and supports and follow through with all issues and needs identified per the ISP.
Lead Internal and External Communication
  • Maintain proactive communication with parents, families, guardians and advocates.
  • Represent the agency in the community or in interagency activities.
  • Facilitate communication to staff, including face-to-face meetings, in order to pass on information and strategy, track goals and objectives, and gather feedback.
  • Cooperate, oversee and work with facilities, landlord and/or property management company employees to build a good working relationship and to maintain a safe, clean and healthy living environment.
