Overview:

We are seeking a dedicated Nurse Practitioner to join our healthcare team. The ideal candidate will have experience working with individuals with developmental disabilities, critical care, geriatrics, urgent care, occupational health, and possess knowledge in areas such as Epic, behavioral health, health coaching, workers' compensation law, and dementia care.

Responsibilities:

- Conduct comprehensive patient assessments
- Develop and implement patient care plans
- Provide primary and specialty healthcare services
- Collaborate with physicians and other healthcare professionals
- Educate patients on health maintenance and disease prevention
- Administer medications and treatments
- Maintain accurate patient records

Qualifications:

- Master's or Doctorate degree in Nursing
- Current Nurse Practitioner license
- Certification as a Nurse Practitioner in the state of practice
- Experience in working with diverse patient populations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic health record systems like Epic
