Open Position: Emergency Assistance Case Manager

Position: Full-time Emergency Assistance Case Manager

Schedule: Monday - Friday, 10:00 AM - 6:00 PM, with nights and weekends available at a higher rate of pay

Location: Chelsea, MA

Salary: $58,000-$60,000, depending on experience, plus up to a $10,000 sign-on bonus after successful completion of probationary period

La Colaborativa is looking for dedicated case managers who will work with Emergency Assistance (EA) shelter eligible families to craft and execute goal-oriented service plans, resulting in increased economic stability, shelter diversion/exits, and health and wellness. This position will operate in a manner that is trauma informed, human centered, and deeply compassionate. Tasked with housing and economic stability case management, the Case Manager will function as a vital link between families, resources, and opportunities. Working independently and as part of a team of human services professionals, this position will play a critical role in affecting the short- and long-term economic and housing stability families facing homelessness.

Key Responsibilities

Bilingual Spanish-English or Haitian Creole-English

Perform client-centered intakes, collect and analyze data, perform triage, and conduct needs assessments to determine short and long term housing, economic stability, social, and health needs;

Develop and implement trauma informed, culturally sensitive housing and wrap around service delivery plans;

Provide case management to a caseload of approximately 15-20 EA eligible households residing in an Overflow Emergency Overnight Shelter facility. Exercising empathy, empower clients through case management and advocacy to achieve tangible results;

Assess opportunities for shelter diversion, as well as housing barriers experienced by clients. Develop strategies to assist clients in overcoming barriers to exiting EA shelter and obtaining stable, safe, transitional and permanent housing. Assist clients with applying for rental assistance including HomeBASE, subsidized housing, affordable housing, and public housing;

Offer resource assistance to clients navigating systems such as public benefits enrollment (eg. DTA, TAFDC, WIC, and MassHealth), childcare, public transportation, medical care, and school enrollment/engagement;

Deliver financial literacy, work readiness, and other trainings to bolster economic stability;

Coordinate educational opportunities for clients including ESOL, digital literacy, and workforce readiness programming;

Facilitate meaningful referrals to specialized services including immigration legal services, mental health counseling, and youth and family services. Work closely with partner agencies to deliver impactful and coordinated services;

Manage cases independently, as well as part of internal teams and external coalitions, in conjunction with community based partners;

Foster and maintain robust partnerships with community based organizations active in the housing field, in order to enhance the quality of service for clients;

Empathetically resolve conflict, promote social cohesion and wellbeing, and connect clients to integrative mental health, physical health, and specialized community-based services;

Advise leadership on emerging trends as it relates to family needs, systemic issues, and barriers faced by families;

Carry out all responsibilities with the utmost integrity and honesty;

Maintain an accurate understanding of the changing landscape of EA regulations and policies;

Perform data collection, prepare regular reports, and routinely update leadership in a timely manner;

and Prioritize cultural safety and sensitivity, specifically with Latinx and immigrant communities, in all facets of work;

and Other duties, as assigned.

Qualifications and Skills

High School Diploma or GED;

1 years of customer service experience, preferably in human services possess demonstrated commitment to customer service;

Must be over 21 years old;

Knowledge of Massachusetts’ EA program policies or public benefits enrollment, a plus;

Experience working in immigrant and multi-cultural communities;

Demonstrable track record of working independently and dependably completing assignments;

Highly skilled at written and oral communication, as well as tactfully interacting with people through a trauma-informed, culturally sensitive approach;

Excellent ability to carry out responsibilities independently as well as collaboratively as a team;

Capable of being adaptable, agile, and proactive;

Detail oriented, methodical, and organized, with excellent time management and organizational skills;

Experience working in an innovative manner, tackling challenges and assignments in unconventional ways or on tasks that require creativity;

Background in excelling in a fast-paced, dynamic environment. Experienced at creating and implementing systems to remain organized and complete tasks on-time; and

Empathetic and compassionate, with a strong sense of community.
