Definition of Position

Program Specialist Residential Supports

Oversees the daily programming for Individuals’ living in Residential Community Home(s). Ensures compliance with Chapter 6400 Regulations; the Allegheny Department of Human Services/Office of Intellectual Disabilities; standards for the health safety and welfare of the individuals supported by direct care employees in the home. Reports to the Residential Manager.

Requirements:

  • Ensures the Organization’s vision and mission statement and the Department's goals and objectives are incorporated in the daily operation of the home
  • Ensures compliance and implementation of the Department’s Policies and Procedures relative to Community Homes. Assembles maintains and monitors all in-house and individual records. Assists with and ensures the upkeep and maintenance of the home.
  • Works all assigned schedules, including but not limited to: arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule, along with serving as an On-Call supervisor per schedule
  • Clocks in and out before and after every shift and submits.
  • Completes and maintains all required State mandated training requirements annually
  • Participates in various committees or workgroups as approved
  • Attends meetings and training sessions as required.

Programming:

  • Participates in the individual plan process, development, team reviews and implementation in accordance with chapter 6400 regulations
  • Provides and supervises activities for the individuals in accordance with the individual plans.
  • Ensures and supports the integration of individuals in the community.
  • Ensures and supports individuals’ communication and involvement with families and friends.
  • Coordinates transportation to and from activities
  • Ensures that individuals live in an environment which promotes their growth
  • Ensures each individuals’ rights are protected

Communication:

  • Informs the Residential Manager and Team Leads of any health, safety, and welfare issues.
  • Reports all medical-related incidents to the Residential Manager
  • Communicates and informs the Residential Manager of operation supports, including fiscal and personnel-related issues on a regular basis
  • Ensures effective communication with the individuals supported, families, and other people involved with the provision of supports for the individual
  • Represents KZL Agency in a professional manner to families, support coordinators, State officials, and community partners when required
  • Maintains individual confidentiality

Documentation:

  • Responsible for the completion of Pre-Assessment and Assessments upon individual’s admission and annually thereafter
  • Monitors and implements Individualized Support Plans
  • Responsible for monthly/quarterly reviews; responsible for development of goal plans with individual goal plan
  • Ensures completion of all documents and all program data, including but not limited to: ISP data, individual attendance
  • Ensures that all documentation is completed accurately and in a timely manner
  • Ensures that all documentation is uploaded and correct in THERAP
  • Reviews, acknowledges, and completes communication documentation including but not limited to: T-logs, S- comms before, during, and at the end of every shift
  • Ensures accuracy and completion of required documentation in compliance with State regulations, Company standards, and Policy and Procedures manual
  • Ensures the accuracy of program, history, and medical documentation
  • Ensures compliance with all state regulations and company policies
  • Identifies areas of concern and/or reports to Residential Manager
  • Maintains awareness of new developments and current trends in the area(s) of expertise
  • Acts as an on-call person
  • Attends required meetings and mandatory training sessions related to Human Services
  • Assumes other responsibilities as required

Staffing Responsibilities:

  • Ensures that all work-related information is disseminated properly
  • Ensures staff has the resources needed to provide social, emotional, and environmental supports to the individuals to ensure their happiness, health, and safety
  • Ensures that staff has the resources needed to provide supports to individuals in activities of daily living

Teamwork:

  • Treats all individuals and coworkers with dignity, respect, and fairness
  • Works as a team to ensure all job responsibilities are complete
  • Provides support to other team members

Qualifications:

  • Bachelor’s Degree in Human Services/Social Services and 2 years of work experience in supports to people with intellectual disabilities

OR

  • Associate’s Degree or 60 Credits hours and 4 years of experience working directly with persons with intellectual disabilities
  • Excellent interpersonal and organizational skills
  • Strong written and verbal communication skills
  • Knowledge utilizing Microsoft Word, Excel, PowerPoint helpful
  • Valid Pennsylvania Driver’s License
  • Reliable transportation
  • Proof of vehicle insurance and registration
  • Satisfactory completion of a criminal background investigation 2021
