KVC Health Systems is Hiring a Case Manager Near Kansas City, KS
Job Description
SCHEDULE:
- Monday-Friday: 9 am-5:30 pm
- One evening, flex day a week: 11:30 am-8 pm
- Must be able to commute to Kansas City
ESSENTIAL DUTIES:
- Manage client intakes and department collaboration.
- Collaborating with Public School Districts with:
- IEP Planning, School enrollments, and transcripts
- Complete compliance reports.
- Ensure licensing compliance.
- Review variance and incident reports.
- Supervise children's treatment.
- Develop and apply program schedules and behavior systems.
- Keep in touch with clients daily to ensure they follow the schedule.
- Ensure structure through behavior systems and house expectations.
- Coordinate youth program with agencies and resources.
- Keep resources for identified needs.
- Document community resource usage.
- Complete progress notes weekly and monthly.
- Provide aftercare for discharged clients.
- Plan treatment and discharge coordination.
- Participate in administrative on-call rotation.
PREFERRED EXPERIENCE/SKILLS:
- BSRB licensure.
- Experience in group home/residential settings or as a case manager.
- Supervisory experience in childcare or juvenile justice agencies working with similar youth.
- Proficiency in office technology, such as electronic health records and Microsoft operating systems.
EDUCATION:
- Bachelor’s degree in a human service field (social work, psychology, human development and family life, criminal justice, counseling), nursing, or education.
LICENSURE/CERTIFICATION:
- Valid driver’s license
- Auto insurance
