Nurse Practioner – Thornton

The Practice: A small primary care office looking for a Physician to join an established clinic. This position offers extensive opportunity for collaboration and flexibility to customize the role. We are looking at Part time Mon -Wed to start with opportunity to work some Thursday and Fridays.

Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Community: Located in Thornton, CO the practice is centrally located between Denver and Boulder and services a diverse patient population.

The Position:

• Provides full spectrum primary care.
• Develops and documents a plan of care for each patient, including a complete medical history, a physical examination, diagnosis of the causes of injuries and illnesses, appropriate treatment and/or referral, and hospital visitations when necessary.
• Adheres to departmental policies, procedures and objectives, ongoing quality improvement objectives, and safety, environmental and infection control standards.
• Maintains patient confidentiality and complies with all federal and state health information privacy laws.
• Records complete, timely and legible medical records.
• Open to seeing patients of all ages and providing women’s health services within a primary care setting.
• Comfortable seeing patients with chronic and complicated health conditions.

Qualifications (Education/Experience/Knowledge/Skills/Abilities):

• 1 year of experience in a primary care physician role preferred, but we will have training available
• FNP degree
• Residency trained in internal or family medicine.
• Board certification in family medicine.
• Excellent communication and inter-personal skills
• Willingness to collaborate with other Advanced Practice Providers and MD.
• Medicare/Medicaid credentialing a plus.

Pay: $70,000.00 - $120,000.00 per year
