Job Description:

KPG Home Health is seeking a skilled and compassionate Home Health Registered Nurse to join our dynamic team on a PRN basis. As a Home Health RN, you will play a crucial role in providing quality healthcare visits to patients in the comfort of their homes.

Responsibilities:

• Conduct patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient progress, ensuring accurate and timely reporting.
• Educate patients and their families on proper healthcare procedures and disease management.
• Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.
• Maintain accurate and up-to-date patient records.

Requirements:

• Current RN license in the state of Arizona.
• Minimum of 1 years of nursing experience in home health.
• BLS certification.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a collaborative team.
• Reliable transportation and willingness to travel to patient homes.

Job Types: Part-time, PRN

Pay: $75.00 - $95.00 per hour

Expected hours: 1 – 10 per week
