Education and Experience Required · Must be licensed as a Licensed Practical Nurse in the state of Minnesota. · Must obtain CPR certification within 6 months from date of hire. · Must have a valid driver’s license and use of an insured personal vehicle. · Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors. Must complete and pass a state required background study. · Able to communicate verbally and in writing to the extent required by the position. · Able to physically perform the duties required by the position. Instills mutual trust and confidence; creates a culture that fosters high standards of ethics; behaves in a fair and ethical manner toward others and demonstrates a sense of corporate responsibility, confidentiality and commitment of service. · Direct Client Care – Caring out preventative and rehabilitative nursing procedures as appropriate for the client's care and safety as directed by the Registered Nurse. Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the client's physical or emotional condition. Teaching, supervising and counseling the client and caregivers regarding the nursing care needs and other related problems of the client at home. · Client Records and Billing – Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the LPN and/or HHA per regulations. · Communication – Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.
