Today, Kittson Healthcare is a twelve-bed acute care federally designated Critical Access Hospital and fifty bed long term care facility which also includes a home healthcare agency, 8 bed assisted living facility and an ambulance service. There is a medical clinic located in the complex as well as a satellite medical clinic in Karlstad, Minnesota. POSITION TITLE: Resident Care Coordinator

DEPARTMENT: Nursing Home

SUPERVISED BY: Director of Nursing (DON)

SUPERVISES: None

STATUS: NON-EXEMPT

CLOSE WORKING RELATIONSHIPS WITH: All nursing home staff, Infection Preventionist, Activities staff, Housekeeping/Laundry staff, Practitioners, Residents and Families

JOB SUMMARY:

The Resident Care Coordinator is responsible for overseeing and coordinating the care of residents in a long-term care facility. This role involves working closely with healthcare professionals, residents, and their families to ensure that residents receive high-quality, personalized care. This role will ensure resident care delivered by nursing staff is in accordance with resident care plans, medical directives, and regulatory compliance.

JOB REQUIREMENTS:

KNOWLEDGE, SKILLS, ABILITIES:

  • Proficiently be able to read, write, speak, and understand the English language.
  • Must be able to work independently and be able to work effectively as a team member.
  • Must be able to work with frequent interruptions, maintaining a high degree of concentration.
  • Ability to identify issues or challenges in resident care plans and support in developing effective solutions to address them.
  • Knowledge of federal, state, and local regulations governing long-term care facilities (HCFA, MN Department of Health and Human Services, Board of Nursing, OSHA, OHFC).
  • Familiarity with individualized care plans based on residents’ assessments, preferences, and medical history.
  • Commitment to quality control analysis.
  • Excellent time management of one’s own time and time of others.
  • Ability to participate and self-educate for job related needs.
  • Excellent attention to detail.
  • Commitment to ethical principles and standards of practices in providing resident-centered care and upholding residents’ rights and dignity.

QUALIFICATIONS:

  • Degree from an accredited Registered Nurse program.
  • Current MN Registered Nurse license.
  • Current CPR certification and IV therapy certification with experience.
  • 1-year general experience in Long-term care setting (preferred).
  • Matrix experience (preferred).
  • Must pass MN background check and with finger printing.
  • Complete TB test within the first 3 weeks of employment.

JOB SCHEUDLE/LOCATION(S):

  • Hallock, MN
  • Working hours are flexible between 8:00am-8:00pm.
  • Long-term care setting.
