Overview

Position: Manager of Case Management

Key Responsibilities:

This is a working manager position. Assists the Director of Case Management to achieve department goals. Shares acute caseload when indicated to ensure efficient care management processes and reduction of agency usage. Supervises Case Management (CM), Discharge Coordinators (DCC), and Transition of Care program (TOC).

Minimum Requirements:

  • Education: • Bachelor’s Degree in Nursing
  • Licensures/Certifications: • Licensed and currently registered as an RN in the State of Arizona or Compact Nursing License
  • Experience: Three (3) years or more of hospital fiscal management, CM, TOC, and discharge planning experience. CM and TOC experience required.CM and TOC experience required.

Knowledge, Skills, and Abilities: Demonstrates knowledge of reimbursement methods, Transfer DRG’s, and observation rate.• Must be able to exercise independent discretion and judgment (utilizing approved criteria) in theperformance of the CM and TOC functions.• A team player that can follow a system and protocol to achieve a common goal.• Highly organized and well developed oral and written communication skills.• Demonstrates sound judgment, decision making and problem-solving skills.• Maintains working knowledge of all applicable state and federal regulations, organizational policies,and national standards of practice. 

Preferences:

Efficient with Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting. • Certification in CM is highly desired. • Extensive experience in InterQual or MCG desired. Education: • Master’s in Nursing
