We need the home Health LVN to visit the patients at home. Bilingual a plus.

Job descriptions: Direct patient care, implements plan of care initiated by the RN, provides accurate and timely documentation consistent with the plan of care, assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care, participates in coordination of home health services, appropriately reporting the identified need to the RN and/or clinical supervisor, participate in personal and professional growth and development, performs other duties as assigned by the RN.

Pay: $40.00 per hour

Expected hours: 10 – 30 per week
